Oral surgery, implantology





The implantate:



The implanted materials have to meet the strictest requirements. They must be compatible with the living tissue, cannot have poisonous, allergic or tumorous effect, and must not corrode. They must have proper flexibility, tensile strength, loadability and wear resistance while must be easy to sterilize and easy to work with. The most appropriate implantates, with their properly formed active surface, by being implanted, they urge the sticking of neighborhood bone tissue to the metal. Thus the stabillity of implanted part will enhance and fixation will be excellent. These bio-materials are integrating while implanted into the bone tissue which means that they are capable to join to the bone tissue at the connective surface, without inserted connective tissue.



The material of the implantetes applied in our surgeries is pure, unalloyed titanium which has a layer of titanium-oxide on its surface. They are screwed into the mandible and after the integration (ossyfying) time, these screws will be used as artificial roots where dental plates and arches can be built on with the help of superstructures. Special literature never reported of any allergic reaction or tumorouos alteration in concern with titanium implantates.

Indications:


The application of implantates is motivated by three factors:



  • With the application of implantates the lack of teeth can be replaced more adventagous to the patient (comfort, functional stability, aesthetic stability) than with traditional methods.
  • With the application of implantates the sacrifice of complete dental material can be avoided! (Not necessary to grind complete teeth)
  • In case of patients are not able to wear (either because of personal reasons or disadvantageous clinical conditions) their traditionally prepared prosthesis.


Classification of cases on the basis of lack of teeth:



  • Lack of one tooth: The lack of teeth can be replaced, without the application of implantates, with the grinding of complete teeth besides the site of lack.
  • Lack at the end of line of teeth: The lack of teeth can be replaced, without the application of implantates, only with combined sheet dental plates.
  • Complete lack of teeth: The lack of teeth can be corrected, without the application of implantates, only with completely removable dental plates.


Contraindications:


(Implantation is not advisable temporarily)



  • operative intervention in case of surgery contraindication (for example acute desease, fever, anti-coagulante treatment)
  • bad oral-hygienie
  • unfinished mandible growth (till the age of 15-16)
  • there is inflammation at the site of planned implatation
  • pregnancy
  • two years after ray-treatment of mandibles
  • osteoplasty is needed before implantation


Final contraindications:


(implantation must be aborted)



  • in case of general surgery contraindications
  • alcoholism, heavy smoking, drug addiction
  • system diseases, immunity disturbances
  • specific infections
  • unfavourable anatomy conditions
  • osteomyelitis of mandibles
  • chronic oral mucous membrane decease


We would like to emphasize that in case of some contraindications, exception can be made, for example in the case of heavy smoking. Namely, in this case, a slower recovery is expected and complications have a higher probability! In the same time maintened diabetes or anti-blood-coagulation treatment is not a disqualification to an implatation operation!

The course of implantation:



1. Preliminary examination and planning



Our patients, registered for implantation, will take part on a free explorative consultation, when anamnesis will be taken (where contraindication deformations should be excluded), and the clinical exploration, and the necessary OP X-ray recording will take place. Study imprint pattern, for planning of operation, will be made during the visit. In case of general status and conditions of the mandible of the patient are proper to the completion of implantation, gnatologue and prosthetic colleagues will be involved into the consultation to form the treatment plan. During the examination, all questions of the patient will be answered then the patient will be enlightened of the opportunities of the operation and of possible complications that may arise during the treatment.



2. Operation of implantation



Following the understanding and signing of the Operational Approval Statement, the patient will be directed to the surgery. Following the local anaesthetic injection, the face and oral cavity of the patient will be cleaned by disinfection liquid. Following the isolation of the operational area and the preparation of the sterile instruments the operation will start. The bone will be examined under the gum of the target area, then the bone nest of implantates will be formed with special drills and implantates will be screwed into the nests. During the operation the patient will not feel any pain, just the vibration of the drill. According to our guests it is less unpleasant than a pulling-out of a tooth. The wound will be closed by stitches that will be removed after one week.



Following the operation a soft-laser treatment will be applied to aid in wound recovery. We provide for all of our guests a special package that contains: after-operation instructions on two pages, antibiotics, pain-killer, wound cleaning tooth-brush, oral desinfectioner, tongue-cleaning brush, dental floss, inter-tooth cleaning brush and freeze-accu. This package will be given to all of our guests.



3. Recovery after operation



In a few days following the operation a series of laser treatment will be provided and it will be controlled that there is no complication around the wound. The most important and the main task belongs to the patient. It is the maintenance of enhanced oral hygienie and timely taking of the prescribed medicines. After one week the stitches will be removed. Following that the body of implantate will recover during the next 3-6 months. The duration of recovery will be determined individualy, on the basis of the bone quality.



4. Prothetical service



Following the recovery time an OP X-ray record will be taken again then following the anaesthetization of gum by a minimal invasion the recovery screws will be removed from implantates and the superconstruction stump will be fixed. Dental plates will be seated on this platform later.



5. After-care and control



Following the prosthetical service patients are asked to come back in determined times to check the oral hygienie and the function and stability of implantate. It should be stressed that successful operation and recovery gives no guarantee for success on the long run! Without careful oral hygienie the patient may loose his/her implantate like his/her teeth beforehand! Instructions concerning that will be provided to each patient of us!
